HEALTH REQUIREMENTS BEGINNING WITH 2020-2021 SCHOOL YEAR
Due on The First Day of School
Incoming IL Pre-Kindergartners
● Complete physical exam within one year prior to the start of school (exam must be completed on required form)
● Up to date immunizations
Incoming IL Kindergartners
● Complete physical exam within one year prior to the start of school (exam must be completed on required form)
● Up to date immunizations
● Must show proof of 2 doses of Varicella vaccine
● Complete eye examination by October 15, 2020
● Dental exam completed by a dentist between October 16, 2019 – May 15, 2021
Incoming IL 2nd Graders
● Must have dental exam completed between October 16, 2019 – May 15, 2021
Incoming IL 6th Graders
● Complete physical exam within one year prior to the start of school (exam must be completed on required form)
● Must show proof of receiving one dose of Tdap vaccine regardless of the interval since the last DTap, DT, or Td dose
● Must show proof of 1 dose of Meningococcal Conjugate vaccine (Menveo/ Menactra)
● Must have dental exam completed between October 16, 2019 – May 15, 2021
Incoming IL 9th Graders
● Complete physical exam within one year prior to the start of school (exam must be completed on required form)
● Must have dental exam completed between October 16, 2019 – May 15, 2021
Incoming IL 12th Graders
● Must show proof of 2 doses of Meningococcal Conjugate vaccine (Menveo/ Menactra) with second dose given on or after 16th birthday
Current Sports Physicals
● Any 4th grader through 12th grader who is participating in a sport must have a current sport’s physical completed prior to the start of practice.
Any student coming from out of state (regardless of grade) must provide a current IL physical and immunization record.
